Do you mean for realistic drawing, or realistic scars in anime? Because anime scars are pretty simple (not easy but simple).

Well for an anime scar usually two lines that curve in opposite directions and run near parallel, then come back to a point. So they start at a point, curve slightly to make an elongated <> shape. Hard to explain in words. It should be pretty strait.

Think thunderbolt but less jagged, and it has tips at both ends.

i think anime scars could be as easy as left to artistic interpretation in design... really, if you draw it, and someone can tell it's a scar, then good job.

Realistically speaking, it would be a play on lighting, very subtle changes in value where the raised skin casts a shadow across the smooth skin thus causing the illusion of a bump... the design again, is up to you unless you're drawing from life, which if is the case than there's no need for your original question but only observation to sate your curious needs XD
